All check requests require a receipt or invoice, which can be uploaded on this form. A screenshot or photo from your phone will usually work.

If your club or team has earned thrift shop awards, the PTSA can pay a vendor or reimburse you for expenses paid. The PTSA cannot transfer money to your club or team account.

Anyone requesting funds from the PTSA must be a member of the PTSA for the current academic year. If you are not a member, please join before submitting this form.
